Position Overview
We are seeking a detail-oriented, compassionate Medicaid Pathways Specialist to serve as the primary liaison between our clients, their families, and Medicaid Pathways for Aging insurance providers. This role ensures efficient waiver processing, clear communication, and seamless coordination so eligible clients can receive timely meal delivery services.

Position Overview
We are seeking a detail-oriented, compassionate Medicaid Pathways Specialist to serve as the primary liaison between our clients, their families, and Medicaid Pathways for Aging insurance providers. This role ensures efficient waiver processing, clear communication, and seamless coordination so eligible clients can receive timely meal delivery services.
Key Responsibilities
- Act as the main point of contact for clients, caregivers, and insurance representatives.
- Navigate Medicaid Pathways for Aging programs to facilitate funding and meal delivery eligibility.
- Manage and track waiver applications, renewals, and approvals.
- Maintain accurate client and insurance records in compliance with HIPAA and organizational policies.
- Collaborate with internal staff to ensure meal deliveries start promptly once waivers are approved.
- Educate clients and families on available benefits and the enrollment process.
- Troubleshoot and resolve any issues related to funding or service authorization.
- High school diploma or equivalent; associate’s or bachelor’s degree in social work, healthcare administration, or related field preferred.
- Experience working with Medicaid, insurance processing, or social services highly desirable.
- Strong interpersonal and communication skills—both verbal and written.
- Excellent organizational skills and attention to detail.
- Ability to handle confidential information with integrity.
- Proficiency with Microsoft Office Suite and database management systems.
