Meal Delivery Driver
We're seeking a compassionate and dependable Part-Time Meal Delivery Driver to help ensure that homebound older adults and individuals with disabilities have access to nutritious, medically tailored meals.
Approximately 75% or more of the driver's time will be dedicated to serving rural clients and communities throughout Allen County, where transportation barriers, geographic isolation, and limited access to services often place residents at increased risk of food insecurity.
A delivery vehicle is provided, so all we need is YOU!
Case Manager
The Case Manager plays a critical role in supporting homebound seniors and individuals with disabilities by providing wellness checks, care coordination, needs assessments, and referrals to community resources. This position helps address the intersection of food insecurity, chronic health conditions, social isolation, transportation barriers, and limited access to supportive services.
This position will transform wellness checks into comprehensive, client-centered interventions that promote health, safety, dignity, independence, and successful aging in place.
